How we got here
Born from personal experience
Rooted in Care NY was founded after watching years of watching individuals struggle alone through a maze of specialists, appointments, and unanswered questions following a serious diagnosis. Despite having access to good doctors, the experience is isolating and overwhelming — not because of a lack of care, but because no one is there to connect the dots.
Those experiences made one thing clear: the healthcare system is not designed to be navigated alone. It takes time, knowledge, and persistence to get the right care — and most people simply don't have all three when they need them most.
We built Rooted in Care NY to be that steady presence. Not a medical provider, but a trusted guide — someone who knows the system, speaks the language, and is entirely focused on helping you move through it with clarity and confidence.

Sara brings over a decade of knowledge and experience working within New York's healthcare landscape. After years of witnessing patients and families fall through the cracks of a fragmented system, Sara founded Rooted in Care NY to provide the kind of personalized, human-centered guidance that changes outcomes. Sara holds a background in patient care and patient navigation, and is deeply committed to making quality care coordination accessible to every New Yorker — regardless of background or circumstance.
